feedback/receipt form?

Hi everyone, hope you all had a great Christmas! I was just wondering if anyone knows anything about the module feedback/receipt form that it talks about in the handbook that we are meant to send in with our work? Does anyone have one or know if we need to send it??
Any help would be appreciated!

    Apparently Avni sent out an email around the 30th of November called important information BAPP. It was on Paula Nottinghams Blog comments under;

    WBS3002 September 2010 entry BAPP - How are you getting on? (18th Dec)

    Yep the e-mail went round on Nov 30th with the receipt form we need to fill out to hand in with our module - let me know if you can't find it, I can always forward it on to you.

  3. Thank you both! I found the e mail and have the form! Do you know if we fill out both parts...office copy and student copy?
